Acne is four problems in one word; light argues with two
Look at what's on the face. Our engineering archive's skin disease atlas describes the primary lesion as a follicular papule plugged with sebum and keratin, bacteria multiplying in the trapped, oxygen-poor space until tissue is damaged. One blockage, four outcomes: comedones, inflamed papules and pustules, nodules and cysts, plus the marks and scars left behind.
The atlas grades by lesion count: under 30 at grade I, 31 to 50 at grade II, 50 to 100 papules and pustules with fewer than 3 nodules at grade III, conglobate cystic acne above 100 lesions at grade IV. That number decides whether they belong in your chair.
Light quarrels with the inflamed lesions and the marks. A blackhead gives it nothing to absorb. A cyst is a medical problem; a dent is a collagen problem.
The mechanism, stated without the marketing
Only the first of three routes is about bacteria. The photodynamic one runs in three steps in our archive's light and tissue interaction volume: Cutibacterium acnes, the anaerobe behind inflammatory acne, makes endogenous porphyrins as it metabolises; light in an absorbed band excites them; the released singlet oxygen kills the bacterium. Porphyrin peaks are listed at 410, 505, 540, 580 and 630 nm, the strongest at the blue end. Hence the blue light in every acne brochure.
Ashkenazi and colleagues showed it in culture in 2003, at 407 to 420 nm. One illumination cut viability modestly; repeats 24 hours apart killed far more. A dish is not a cheek, but that is why a single session underwhelms.
Route two is heat at the pilosebaceous unit, tied in our archive to lower sebaceous activity and better texture. Route three is haemoglobin, peaks at 418, 542 and 577 nm, which fades post-acne erythema, the pink stamp a healed papule leaves for months.
Now the part brochures skip. The 2016 Cochrane review pooled 71 randomised studies and 4211 participants and found the evidence for light therapies in acne short on quality. Useful adjunct, yes. Proven first-line therapy, no.
Filter choice: the glass on the quotation decides the indication
An IPL platform is a xenon flashlamp plus coloured glass, and the glass is the argument. Our device manuals ship the E-Light platform with dual filtration below 400 nm and three working cut-offs: 530-1200 nm for rejuvenation and acne, 585-1200 nm for vessels, 610-1200 nm for hair. A fourth is optional and buyers miss it constantly, an acne cut-off at 430 nm.
Read that against the porphyrin peaks. A 530 nm cut-off never delivers the 410 nm peak, only the weaker 540 and 580 nm ones plus haemoglobin at 542 and 577 nm, so a photofacial pass on acne works mainly through vessels. Want the blue end? Name the 430 nm filter on the order.

IPL, blue LED or ALA photodynamic therapy: which one buys you what
Few buyers really choose between two filters. They choose between a broadband platform, a single-purpose blue panel and a drug-plus-light protocol.

What a course looks like, and what to promise
Our device manuals' parameter sheet tabulates acne marks with the pigment and vascular indications by Fitzpatrick type. First-pulse width runs 6.0 to 10.0 ms at type I, narrowing to 2.8 to 3.6 ms at type V; pulse counts drop from 4-5 to 2-3; console energy steps down as phototype darkens.

Intervals belong to the treating clinician; the shape is fixed, a course rather than an event. Our manuals note lesions surface within 2 to 3 days after an acne pass and settle over roughly 7 to 15 days, and squeezing the softened plugs is forbidden. Say so on the way in, or she does it at the mirror on day two.
Maintenance isn't a sales trick. No filter removes a sebaceous gland, so activity returns and the acne with it. The American Academy of Dermatology says these treatments rarely clear acne alone and that patients usually need follow-ups once or twice a year. Price the course that way. The hard boundary, and the isotretinoin rule
Moderate to severe acne is a medical diagnosis. Grades III and IV, anything nodular or scarring as it goes, anything failing topicals: that belongs with a clinician, and the 2024 American Academy of Dermatology guideline recommends oral isotretinoin for those patients. Our device manuals make an undiagnosed lesion an outright contraindication. Can't name what you're treating? Don't treat it, and refer for biopsy when the presentation is atypical.
Isotretinoin gets its own paragraph because operators keep getting caught. Our device manuals defer treatment for 6 months after the last dose; that is the rule your staff follows. The evidence is softer than the rule: a 2017 JAMA Dermatology review of 32 publications found insufficient evidence to delay hair removal lasers, superficial peels or fractional laser, while still advising against fully ablative work. Treating sooner is the prescriber's call, in writing.
Scars are the other boundary. The atlas puts atrophic scarring after cystic acne at 4.5 to 16 percent, with little tendency to resolve alone. That is fractional work with a CO2 fractional laser system, sequenced under acne scar resurfacing. Settle the disease first.
Screening and aftercare your operator must not improvise
- Photosensitising medication. Our manuals name tetracycline outright, and plenty of acne clients are on a tetracycline-class antibiotic. Ask at every visit.
- Phototype and pigment risk. Our archive warns darker patches can appear weeks later and persist from 2 weeks up to a year, the risk climbing on darker skin. These clients fear that most.
- Test spot first, within 3 days of the session, before you commit a full face.
- Sun. Four weeks clear of unprotected exposure or a solarium beforehand, SPF 30 daily after, no exfoliation for 1 week, no picking.
- Active infection, herpes in the field, pregnancy, keloid tendency, pacemaker. All on the manual contraindication list; radiofrequency adds a metal-jewellery rule.
For grade I and II faces our archive puts a water and oxygen jet first, 50 to 80 micrometre droplets at 200 m/s clearing blocked follicles before any light fires. That is the OJ-01 water and oxygen jet.

Questions buyers actually ask
What can a clinic honestly advertise?
Advertise a reduction in inflammatory lesions and improvement in post-acne redness across a course, with maintenance. No cure, no claim about blackheads, no hint that it substitutes for prescribed medicine. Check local advertising rules: a leaflet that passes in one country is a fine in another.
How many sessions before a client sees anything?
Expect a course, with change building over several sessions, not after the first. The laboratory work behind the porphyrin mechanism showed the same pattern, repeat illuminations killing far more bacteria than one. Publish a course price, never a per-shot promise.
Do I need the 430 nm acne filter, or is 530 nm enough?
For post-acne redness and general photofacial work the 530-1200 nm and 585-1200 nm cut-offs carry it. If you mean to market an acne protocol built on the porphyrin mechanism, the 430 nm cut-off reaches nearer the strongest peak at 410 nm. It is optional, so confirm it on the quotation.
