IPL for Rosacea and Facial Redness: Equipment Demands

IPL for rosacea and facial redness treats the vascular side of the disease and nothing else: diffuse flushing and visible telangiectasia respond to filtered light, papules and thickened nasal skin do not. Oxyhaemoglobin is the target, so the vascular filter, the pulse structure and the cooling on your platform decide whether a cheek clears or complains.

Red faces are two different problems wearing one name

A client points at her cheeks and says redness. She could mean either of two things, and they don't behave alike under a flashlamp.

One is discrete telangiectasia: thread-like vessels you can trace across the nose, the alar creases, the upper cheeks. A normal capillary is invisible and wide enough for a single red cell, our engineering archive notes; alcohol, oestrogen, corticosteroids and years of sun push it open until it reads as a bloodshot line. Papillary capillaries widen to roughly 0.1 to 1.0 mm. The archive puts simple telangiectasia at 15 to 20 percent of people, mostly on skin lighter than Fitzpatrick III.

The other is diffuse background erythema with flushing. No traceable vessel, just a cheek that runs hot and stays pink. Our archive calls that latent capillary dilatation.

Here's the complaint engine, from our archive's clinical notes. Thread vessels lift and darken to a purplish red after a pass, visible in the mirror on the way out. Diffuse redness shows no obvious immediate vessel change, while the skin still reddens and stings. Same session, two very different first impressions. Tell the diffuse-redness client what to expect, or she'll leave believing she paid for a sunburn.

Oxyhaemoglobin is the target, and the vascular filter decides whether you reach it

Blood absorbs. Our engineering archive puts haemoglobin's absorption peaks at 418, 542 and 577 nm and names oxyhaemoglobin as the target chromophore in vascular work. Each peak sits in a band melanin also enjoys. That's the tension of the indication.

Anderson and Parrish set the rule in Science in 1983: pick a wavelength the target absorbs better than its neighbours, keep the pulse inside the target's thermal window, and the vessel cooks while tissue around it doesn't. Tan, Murray and Kurban measured the trade six years later. Vascular injury reached roughly 0.5 mm deep at 577 nm, 1.2 mm at 585 nm. Give up a little absorption, buy depth.

That's why a vascular filter exists as a separate part. Our device manuals ship three cut-offs on the E-Light platform: 530-1200 nm for rejuvenation, 585-1200 nm for vascular lesions, 610-1200 nm for hair. Arrive with the hair and rejuvenation glass only and you don't own a vascular machine. You own one missing a cheap part. Other cut-offs work too. The point isn't one magic number; it's that the filter set has to be a named line on the quotation.

Pulse structure and cooling: where a cleared cheek becomes a bruise

A rosacea cheek isn't one target. It's a spread of calibres, and our archive ties thermal relaxation time to the square of target size: a 1.0 mm vessel holds heat roughly a hundred times longer than a 0.1 mm one. No single pulse length suits a whole face.

Pulse trains answer that. Our archive is direct: more sub-pulses means more heating at depth with less provocation of the epidermis, and a wider pulse acts more gently on deep targets. Stability matters as much as shape. The archive keeps traces of a regulated square-wave train holding peak across every sub-pulse, and of an unregulated one whose peak decays inside a trigger. If peak sags, the energy you dialled for the tail punished the opening pulse.

Now the counter-intuitive bit. Our archive states that simple telangiectasia generally does not want aggressive epidermal cooling, because those vessels sit close to the surface in the papillary layer; over-protecting the epidermis pulls heat out of the target and can leave the session clinically useless. Shorten cooling time, or moderate the window temperature. Port-wine stains and haemangiomas are the opposite: higher fluence, mandatory cooling. Anyone who says colder is always better hasn't read the same material.

Risk sits on the other side of that dial. Purpura, blistering and pigment change are recognised outcomes, post-inflammatory hyperpigmentation takes months to fade, and eye protection must cover the full broadband output.

Broadband IPL or a dedicated vascular laser?

The answer turns on your case mix, not on physics. The 2019 National Rosacea Society management update lists intense pulsed light beside the narrow-band vascular lasers, 595 nm pulsed dye and 532 nm KTP, among the options for vascular signs, and the American Academy of Dermatology's public page treats lasers and lights as one option set, not a ranking.

Broadband is the better buy when redness is diffuse background erythema across whole cheeks, when your book mixes pigment with vascular in the same hour, and when one room has to earn across several indications. Big filtered spot, fast area coverage, other jobs between clients.

A narrow-band vascular laser suits the opposite practice: discrete large-calibre vessels, resistant nasal telangiectasia that returns after a course, vascular work and little else. One band, one job, more precision per vessel. Buy it to carry a general aesthetic menu and you've bought an expensive specialist.

What to verify before you advertise rosacea treatment

Send this to the supplier in writing.

  • Is the 585-1200 nm vascular filter in the crate, or an option you assumed was standard? Get it on the packing list by name.
  • Do pulse count, sub-pulse width and delay set independently, or only as soft, medium and strong with the numbers hidden?
  • Is pulse-to-pulse peak regulated? Ask for the trace, not the claim.
  • Can contact cooling be dialled down or off? Shallow vessels need latitude.
  • Which spot sizes ship? Our archive lists 10 x 10 mm, 8 x 40 mm and 15 x 60 mm sapphire windows, larger spots reaching deeper at equal energy.
  • Rated lamp life, and the price of a relamp? Facial courses are shot-hungry.
  • Does your market's registration cover vascular claims, and whose name is on it?

Read the last row twice. The direction of the evidence is good and its quality isn't, so the consent form should promise gradual improvement over a course and nothing sharper. New vessels forming isn't failure. It's the disease continuing, and maintenance once or twice a year is part of the deal, not an upsell.

The medical boundary you don't get to blur

Rosacea is chronic, and diagnosis is a clinician's job. The National Rosacea Society Expert Committee moved the field to phenotype-based classification in 2017, and its 2019 update places light and laser therapy alongside topical and systemic medicine. A red face is also the presenting sign of things that aren't rosacea: lupus, photosensitivity reactions, seborrhoeic dermatitis. A persistent solitary lesion belongs with a doctor who may want a biopsy.

Papulopustular disease is not an IPL target. Papules and pustules need medical management, phymatous thickening is ablative or surgical territory, ocular involvement goes to ophthalmology. Screening still applies: recent UV exposure including self-tan, photosensitising medication, recent isotretinoin, active infection in the field, pregnancy. Triggers keep working after you treat, so sun protection does more long-term than your fourth session. FAQ

Can we advertise this as a rosacea cure?

No, and the claim costs more than it earns. Rosacea is chronic. Filtered light reduces visible vessels and background erythema over a course, with maintenance afterwards. Most markets police claims about a named medical condition, so check your registration before rosacea goes on a price list.

We already run pigment and hair. What's the smallest upgrade that adds redness work?

Usually a filter and a parameter table, not a console. If your platform takes the 585-1200 nm cut-off and lets you set pulse count, width and delay independently, you're close. Check two more things: whether cooling dials down, and whether fluence has headroom at cheek settings.

Should we take type IV and darker clients for facial redness?

With caution and a test patch, knowing competing melanin narrows the working window sharply. Energy comes down and pulse width shortens as phototype climbs. Post-inflammatory hyperpigmentation is the realistic risk, not a burn. A tanned type III behaves darker, so wait for the colour to fade.

What disappoints buyers most after the sale?

Locked presets. A console with three intensity buttons that hides pulse count, width and delay is fine for hair and hopeless on a face where calibre varies across one cheek. Runner-up: finding the vascular filter was an accessory line nobody ticked.
